Step 1:
Fax this completed sheet to (856) 848-0937 or call (800) 257-5540 ext 2322 to receive an Implementation Guide. A Checkpoint Source Tagging Trade Development Manager will follow up on your specific needs and to answer any questions.
Step 2:
Send Checkpoint one (1) shelf-ready sample of the requested product(s) for source tagging and a copy of this form. A purchase order or credit card number to bill for the product evaluation service will need to be sent to Source Tagging Customer Service and entered before any service can be provided. Product evaluations for up to five (5) SKUs/products per submittal will be charged $400 (Part #7350371).
Customers will receive:
- A product evaluation report with digital photos advising proper placement location of the Checkpoint label.
- Return shipping at Checkpoint’s expense via ground shipment.
- One follow-up phone call by a Checkpoint engineer to review Product Evaluation and recommendations for application methods.
